I had the same problem. Check that it is saved as a .tga file and also saved in 32-bit if you saved it through paint.net

Check if you selected the right minishirt in the custom team folder... If you chose the first minishirt and the jersey isnt in the c00 folder, than the shirt is in the wrong folder...
Otherwise you could check the size of the jersey. It should be 455*313 pixels. Anything less than that and it will not work!

If it looks like that, the game can't find the shirt file = it's not named correctly or not placed in the correct place.
