What exactly do you want? If you want to make team specific leader shirts, the templates are the shirts themselves. You just need to paste your team shorts and add the transferts. You could also use Mithrillian's template pack, which (for leader shirts) is about the same as doing the above. But I'm not sure if this is what you want.

Do you have the original file of the jersey? That would.be .pdn or so for paint.net?
EDIT: the files in my pack are xcf and thus for use in GIMP. When I get back on my computer I will make a paint.net version of the pack
