Last night I just figured out how to put a custom jersey into pcm 11. But when i started a career and started playing, my team with custom jerseys did not have a helmet. So how do I fix this? Also when I have the yellow jersey (for example in tdf) I don't have a sponsor on the leader jersey, how do I fix this?
The helmet thing: choose a helmet in the pre-race equipment selection menu
For the sponsor on the leader jersey: you need to have transferts, which should be placed in the same folder as the shirt. If you don't have them, you can post the shirt and ask a graphics guy to do them (quite simple to do).

But since I make new careers all the time, I want to learn how to make transferts and to make equipment And since you had such a good answear to my question, I will bring you another one. When I put the custom jersey into the game I only replaced it with the jerseys that you can choose when you make a career. So how do I put my team in the database, and how do I get sponsor picture like this one:: in the career?
Well, how to make transferts is pretty much how to make shirts, which you can find some tutorials out there, only changing the template (from shirt templates to transfert templates). You can find transfert templates here. Equipment is considered by most harder to make (which explains why there are few equipment makers), but it's about the same thing the other: creating textures for models (frames/wheels/helmets). In this case the templates are the _d files of each equipment themselves. Basically for doing these 3 you need graphics knowledge (and of course an image editing software, the knowledge alone won't be enough ).
And lastly, that's the team logo in the image. You can find a template for it in the same file as the transferts, and where to place it is explained in the link above.
