Yes. The two ProContinental teams with the most CQ points (Superprestige ranking) at the end of the season promotes.

Last 2 WT teams in the World Tour ranking are relegated. And, as pellizotti said, Top 2 Continental teams in the CQ ranking are promoted.

Btw pellizotti, it's the PCM11 forum, so there's no Pro Continental Pfft
