Basically, you'll have to add the files to Program Files/Cyanide/PCM12/3D/Cyclists/Equipments/Frames or Helmets or Wheels, and then edit the DB which is the hardest part.
On editing the DB, this is a good start, but might be outdated in some parts:

Then KenL did this updated version of fenian's tutorial for PCM11 and 12, all with pictures. However the pictures have gone offline and the download link too, so you should perhaps try contacting him through PM if he still has the tutorial:

Additionally, if you a the PCMDaily DB, it's very simple to add equipment for teams: for example, if you have a frame for Colombia - Coldeportes, you just need to rename it for 2012_frame_colombiacoldes and replace the old files in the directory I mentioned in the first line of this post.
If you want to add equipment that do not belong to teams (in the PCMDaily DB), you can use the "custom" equipment spots (rename them "2012_frame_customX", where x is a number from 1 to 5 corresponding to the equipment of same number in the equipment selection menu in game), or "custom_helmetX" if it's a helmet or "custom_wheel_X" (in this case only from 1 to 3) if it's a wheel. Remembering that you can check all equipment file names by opening the DB, table STA_equipment.
