Put it in PCM 2012 -> Gui -> minishirt under the same 3 letter initials as your jersey name. so if you are using custom team 1, it would be c01_minimaillot

I use a program called blender, and there is a template pre-made.
The only work necessary is to pick the directory of the jersey, convert from .tga to .dds using paint.net, and upload it to here!
