That's sponsor difficulty. It decides how big your budget is.
The actual race difficulty is found in the options menu in the top right corner.

Those are not the gameplay difficulty, that's the financial difficulty. The gameplay difficulty (which can be changed at anytime and applies to all games - single player, career, etc) is changed in the Options menu.

The difficulty you choose when starting a career is for the finances.
The gameplay difficulty is in the options. There you have also extreme.
Edit: Wow. I`ve been Zabel`d twice...

Easy: budget bigger than riders salaries.
Normal: budget equal to rider salaries.
Hard: budget lower than rider salaries.
I don't know if it has an effect on sponsor goals, never heard of it (even because the goals barely matter).

Hence my edit.

Many people I think think when switching too extreme will actually make the AI much more smarter in some way, which is not the case
Extreme is my choice. I think the only unfair thing about it are the flat prologues which the boost to the AI stats is quite significant and you can't really do much on such short distance, hence in general poor results.

Most generally i'll play on Hard, but if i have a poor team and it's a hill/mountain stage i may switch to Normal to limit time losses, or if i have a super team and its flat, i'll switch it to Extreme, even though i still win. I won all 3 flat Paris - Nice stages with Robert Hunter on Extreme difficulty |

I would say everyone has to play on Normal or Hard.
Only newbies on Easy and the most experienced players ever on Extreme.

Normal For Me.It depends how you set out.If you start with a low ranking team(lowest in my case)playing in hard mode makes it almost impossible to secure wins/promotion cause ur rider stats are not good to compete and it can become a tiresome effort. On the contrary if u start with a big team normal mode makes the game too easy n boring cause u will win most races.
So its basically a simple equation: Normal for Lower ranked teams/inexperienced players,Hard for bigger teams/experienced players.
I switch from normal to hard when I have good enough riders to compete with big teams. |

I did two posts about this in the pCM11 forum, where I found the difficulty "Hard" way too easy. "Extreme" is the only one that suits me
on the other hand, I´ve been playing this game for six years so I´ve learned a few things. |

Play on Hard, Used to drop back to Normal for Spints (as took me ages to get them right) but now on Hard for all.
How does the factoring work, on extreme do all your riders get worse and theres better. I didn't notice much difference between Normal and Hard, just less tolerent of mistakes.

Your rider stats are always on 100%. Then, for AI riders:
Easy: 98,5% ? (can't remember exactly and can't check either)
Normal: 100%
Hard: 102,5%
Extreme: 105%

the difference from hard to extreme is, for me, mostly that it is really hard to win top races. I remember 2011 PCM I had really perfect form for both the Giro and the Tour (83 in MO for both my riders) and both of them didn´t win the tours. So sometimes the AI is simply better even if your rider has 96-99 in fitness and the opponent has the same stats. (according to lluuiigis post above the precentage explains it) annoying, but at the same time I don´t want to play on Hard since I win so many races that it becomes unreal. I want it to be more like the reality. |

I play on extreme, as won 3 GT's in a year on hard so I decided to go up a level. Tbh there isn't much difference between Hard and Extreme but I find it more interesting |

Well, made the permanent switch to hard. Used to switch between Normal and Hard but actually you learn much more when playing on Hard, you get much better at the game! |
