The promotion ranking for all divisions is based on the strength of all teams at the end of transfers. The 18 teams with the best riders are in WT, next 18 (or 16 or 20? I don't remember) are in PT and the rest in CT. So, it has nothing to do with results, just this Team Index.
I've looked into the team index a few years ago and it wasn't super clear how it was calculated. It wasn't just straightup the AVG of the Top X riders, so it is probably weighted in some way. And certainly not all riders count. So, get strong riders and you'll eventually promote.

basically, your first top 5 riders count towards your rank power.
it is a terrible system, but thas't what we have... (during my career in 3rd season an asian team got promoted to world tour, having 8 riders in total... had to relegate them manually to pro conti due to an insufficient number of cyclists...
also development teams can get promoted to pro conti, another mishap.
still, uci rule states that pro conti teams can have between 16 and 25 riders,
while continental teams should have 10-16 riders roster.
for world tour limits are 23-30 riders.
