Nibali might have refused to take it, but with 3-4 days left he knew he had to go for it. One could see the bosses in Astana nodding and recognizing Yes it worked. Only way to turn this around.
And me who almost never speak about this hidden stuff....  Honestly I doubt Nibbles was out his juice during the main part of the Giro. The circumstances have just come together for him. Motivation seems to be the key!
