In short: Chaves and Orica started the chaos up the first climb and completly blew up on pack. Chaves, Nibali and Kruijswijk were all that was remaigning at the top. Kruijswijk tried following but crashed early in the descent.
Then kruijswijk lost his calm unfortunatly and worked too hard afterwards and tada. His lack of team finally showing.

Got to be fair with Nibali, he always attacks even if he doesnt have the legs, like Contador, they are real entertainers...
