One of the "usual" Nations Databases which make it possible to play with national teams instead of the normal sponsor teams.
Installation: Extract the DB to My Documents/Pro Cycling Manager/Database and make sure to select it in-game in Options -> Database.

Interesting idea, but not well-made... There are countries, as well as regions in this DB, but they're sometimes completly random. For example - Portugal, Denmark or Poland are divided into two teams - and always Team A are riders from one RL team (e.g. CCC Polsat, LA-MSS, Designa Kokken) and Team B are riders from another RL team (DHL-Author or Liberty Seguros) plus few from other world teams...
In one word - great idea, but riders are randomly spreaded out into regions.
