So the game has 4 difficulty levels: Easy - Normal - Hard - Extreme. I wonder what part of gameplay do the levels affect. Is it the behaviour of the AI, budget, rider management...?
I haven't found the info and am too lazy to test it, so would appreciate anyone's imput on this.

Exactly this. Difficulty only pertains to 3D races, nothing else. Simulating basically RNGs a winner based on the riders stats and form in relation to the profile of the stage. None of the other career mode features have a “difficulty” aspect.
To go a step further, that multiplier works BEFORE the CPU rider’s form is factored in too. So a rider with an attribute of 80, for example, rides like it would 82 on hard, or 84 on extreme.
Personally, I find hard can still be too easy, and extreme can crater realism by making riders who wouldn't otherwise be contenders, possible stage or GC winners if they are great form (this is most evident in TTs). I’ve found a sweet spot at modifying the hard or extreme value to be 1.0375, which can be done using a DB editor. It gives a healthy competitive boost, but doesn’t quite allow for a rider to come out of nowhere with far lower stats.
All that being said, as with any game, difficulty it purely personal preference, so go with fits your play style the best!
