They aren't stored individually (anymore). You find them all with a lot of other UI stuff in just one - or rather, four - tga files and the countries are named in the corresponding xml files. You find them directly in the Gui folder.

No idea. Theoretically there's room in the tga and a way to define a position in the xml file.Safer would be to replace an existing country and flag. Is only the Kosovo flag missing, though? Are there riders from Kosovo in the game? If not a flag alone won't help and more serious editing would be required, you'd have to add a new region, here's a bit about that:

In PCM2020 would one then have to add the flag of the new country in all four tga files in the Gui folder? (PCM2020 does not have the "country_flags1080.tga" file as in PCM2015). |

The safe answer would be yes. If it's just for yourself you could probably ignore the resolution you aren't using.
