Well basically you have to save the shirt in tga format and replace the file you want.
If it is a custom team, go to my computer -> Hard Drive (often C -> Program Files -> Cyanide -> Pro Cycling Manager -> CM_Textures -> cycliste -> team -> c00.
In game when you select a jersey it may have the minishirt of the original custom shirt, but in-game it will be your new one.

@joselesm Here is the shirt I "Philippines" isn't in capitals, but I found a picture of the name on the internet and it looked good.
