Okay, well first of all, are you using this for a custom team? If you are, then you need to rename all the files that have the che_ at the beginning to c00_. After you have done this, place the files that ARE NOT IN A FOLDER IN:
PCM 2012 --> 3D --> cyclists --> cloth --> team --> c00
overwrite the files already in there.
Next, place the team & team_shirt in the folder
PCM 2012 --> Gui
overwrite everthing in those files.
So when you are finished, you should no longer have anything with a che in it it should be c00.
Start your career, and when you start, your minishirt should show up in the top right corner, which means everything worked great!!
The minishirt showed up when I went to create my team, so it looks like it worked 
I didn't notice the US champion jersey though... are you sure it was included in the download pack?

Here you go, just put it in with the rest and replace the che_ with c00_

That's a template for creating Polish national champion's jerseys. It wont look very good if you use that as a jersey by itself, since there's no plis and no pants.

I think you still didnt really understand his request.
He says that he wants you to do a shirt which the Polish Cyclists are using at the Worlds (National Cycling team).
This is the Current one:
As you can see its plain and simply boring! What Kadz wants you to do is to create a jersey which the polish cycling federation will use in the worlds (for PCM obviously)! So basically a remake or alteration of the shirt posted above...
I hope it is clear now what he wants because if not, then I question A: My explanatory skills and B: Your understanding of the Words Polish National And Cycling Team
