Rui Alberto Faria Da Costa is his full name. Hence Rui Costa would be the Anglicised shortening of it. I don't know if that is the Portugese convention though.

It is, but we don't say it simply because:
A. We feel there's no point
B. unlike the "van", it's not considered part of the last name, it's sort of a connection bridge
